Insurance Info

 Because insurance is based on a sick care model & only pays out for disease care, insurance generally does not cover functional medicine services. For those who are residing in Colorado, we can accept your FSA/HSA card and we can provide a superbill for you to re-submit to insurance. Health Model focuses on treating the root cause, building a strong & healthy foundation, & creating long-term sustainable results that are tailored & personalized to you.

Whenever possible we will guide you to get labs through your primary care provider so you can run that through insurance. Our clients get DEEP (at cost) discounts on supplements and functional labs.

We do offer discounts for military, first responders, teachers and pastors. We also offer a referal bonus program for any friends and family you send our way.
